Important: Insurance coverage and participation can change periodically. In addition, some insurance companies offer multiple plans, and our office may not participate with every plan offered by a particular insurer.
We strongly recommend that patients contact their insurance provider directly to verify that our office is in-network with their specific plan prior to their appointment. Patients are responsible for understanding their coverage, benefits, and any referral or authorization requirements.

Do you accept all plans from these insurance companies?

Not necessarily. Many insurance companies offer multiple plan options (such as HMO, PPO, EPO, marketplace plans, employer-specific plans, or narrow network products). Our participation with an insurance company does not guarantee participation with every plan offered by that carrier.

For this reason, we recommend contacting your insurance company directly to confirm that our office is in-network with your specific plan and to confirm your individual benefits.

Why should I verify my insurance before my visit?

Insurance participation and coverage can change periodically. Verifying your coverage ahead of time helps avoid unexpected charges and ensures your visit is covered according to your plan benefits. While our staff may assist in submitting claims, verification of benefits is ultimately the responsibility of the insured.

We recommend confirming the following with your insurance provider prior to your visit:
• Whether our practice is in-network for your plan
• Whether your visit requires a referral or authorization
• Your expected out-of-pocket costs
